The quickest way to get a copy of the application. You can view,
print, and (with the Word version) complete the application directly
from your computer.
The application consists of six parts:
- Basic Application for College Admission. You should complete this five-page form providing basic biographical, educational and extracurricular information. You will also indicate to which college(s) you wish to apply.
- Personal Essays. These essays should be carefully and thoughtfully prepared and typewritten on separate sheets of paper. Each page should have your name printed at the top.
- FAFSA4caster Financial Information. You and your parent(s) must complete the online FAFSA4caster and submit printed copies of the results. FAFSA4caster uses figures from your 2007 Federal income tax returns to provide you and Act Six with an early estimate of your Expected Family Contribution (EFC).
- School Report, Official Transcript, and Exam Scores. You should give this form to a school counselor who can obtain an official transcript and who has access to exam results as well as school and class GPA information. All materials should be returned to you in a sealed envelope.
- Academic Recommendation. You should give this form to a teacher who has taught you an academic subject. The form and accompanying letter should be returned to you in a sealed envelope.
- Community Recommendation. You should give this form to a pastor, mentor, employer, or other person (possibly at school) familiar with your leadership potential and involvement outside of school. The form and accompanying letter should be returned to you in a sealed envelope.
In addition, applicants who wish to be considered for admission to Northwest University must complete the:
- Northwest University Supplement. To be considered for admission to Northwest University, you must complete these supplemental requirements by writing an additional essay, obtaining your community recommendation from a pastor, and affirming Northwest’s purposes and standards.
